Abstract | 「抄録」人口の高齢化とともに認知症の患者数は増加の一途をたどっている. 認知症を有する高齢者では睡眠-覚醒障害と随伴する精神行動障害の頻度がきわめて高く, 難治性であり大きな介護負担をもたらすため, その対処は医学上の急務となっている. 認知症の睡眠-覚醒障害は診断および治療がむずかしい. 不眠症, 睡眠関連呼吸障害, 睡眠関連運動障害, 睡眠時随伴症, 概日リズム睡眠-覚醒障害など, その内訳はきわめて多岐にわたるほか, 午睡によって睡眠恒常性が破綻して, 結果的に夜間睡眠の質が低下していることも少なくない. その診断には昼夜の視点から症状を診立てる必要がある. これらの睡眠-覚醒障害の多くは不眠症状を呈するため安易に催眠鎮静系薬物を投与されることが多いが, 効果が乏しい一方, 副作用が出現しやすい. そのためリスク・ベネフィット比を勘案しつつ, 睡眠衛生指導や時間療法などの非薬物療法などを効果的に取り入れていくことが必要になる. |
