Abstract | [Points]●グレープフルーツジュースは小腸のCYP3A/P糖タンパク質(P-gp)を阻害し, 基質薬物の血中濃度上昇を引き起こす. ●グレープフルーツジュースによるCYP3A/P-gpの阻害はコップ1杯の量で引き起こされ, 24時間〜3日間阻害効果が持続するため, 相互作用は服薬時間をずらすことでは回避できず, 事前にグレープフルーツジュースの飲用を避けるよう説明が必要である. ●グレープフルーツジュースならびにアップルジュース, オレンジジュースは小腸のOATP2B1を阻害し, 基質薬物の血中濃度減少を引き起こす. ●フルーツジュースコップ1杯(250〜400mL)で十分にOATP2B1は阻害されるが, 約4時間あけることで阻害の影響がなくなり, 相互作用の影響を回避できると考えられる. |
