Abstract | 「Abstract」脳卒中患者の効率良いリハビリテーションのために予後予測は必須である. 発症前の日常生活動作(ADL), 年齢, 併存疾患などの予後規定因子に併せて, 脳画像所見, 片麻痺などの症状の経過を勘案し, 予後予測を行う. 脳卒中病巣の影響は, その大きさだけでなく, 部位を勘案して評価すべきである. 大脳半球に病巣のある症例の回復過程は対数曲線的である場合が多い. 食事・整容・更衣・移乗・歩行など, ADLには項目別に難易度がある. これにより経過に応じて訓練すべき課題, 機能予後の具体像がより明瞭となる. 「二木の予後予測法」は基礎的ADL・年齢・症状の経過より予後予測を行う臨床上有用な手法である. MRI拡散テンソル法脳画像の最新の研究は, 錐体路神経線維のワーラー変性の指標となる発症2〜3週頃の大脳脚fractional anisotropy値と, 回復期リハビリテーション病院退院頃の片麻痺症状, ADL指標が中等度以上に相関することを報告している. より正確な予後予測のためには, 上述の予後規定因子・脳画像・症状を総合的に組み合わせて勘案することが望ましい. |
