| Abstract | 「Abstract」関節リウマチでは, 腱鞘滑膜炎, 手関節周囲の変形, 尺骨頭の相対的な背側亜脱臼変化による機械的な摩耗などにより手指伸筋腱の皮下断裂を時に合併し, 手指の伸展不全を呈する. 手指の伸展力の回復を得るためには外科的な伸筋腱再建術が必要である. 伸筋腱の再建は, その断裂した腱の本数や部位に応じて, 端側縫合, 腱移行, 腱移植を適宜選択し, 時に組み合わせて行う. 後療法には減張位早期運動療法, dynamic splintや外固定を用いるものなどがあるが, 腱の再建法により選択すべき方法は異なる. また個々の患者の状態に合わせた判断も必要である. なお伸筋腱の再建時には, 手関節の形成術や固定術を同時に行うのが普通であり, それに対する後療法も併せて行う必要がある. 本稿では伸筋腱再建術の術式の概要と, それらに応じて選択すべき後療法やプロトコル, 注意事項について解説する. |
