Abstract | 「はじめに」電解質異常は直接的・間接的に腎障害, 特に尿細管間質障害と関連しており, さまざまな病理学的変化あるいは多尿をはじめとする尿細管機能異常をきたす. 本稿では, 特に低K血症と高Ca血症を取り上げ, その病態生理とそれによって生じる腎障害について概説する. 「I 低K血症」「1. 低K血症の病態生理」低K血症の原因は大きく, 細胞内への移行, K摂取の低下, Kの喪失(腎からの喪失, 腎外喪失)に分けられる. 細胞内への移行には, インスリン過剰, β刺激薬などの薬剤性のもの, 家族性低K血症性周期性四肢麻痺, 甲状腺機能亢進症がある. 腎からの喪失には, 薬剤性(利尿薬など), レニン-アンジオテンシン系(renin-angiotensin system:RAS)の亢進, 尿細管機能異常症などがある. Bartter症候群は, ヘンレの太い上行脚におけるNa再吸収に関わる分子(Na+-K+-2Cl- cotransporter, ROMK, ClC-Kb, barttin)のいずれかの異常によって生じる. |
