| Abstract | 「はじめに」Down症候群の合併症の一つとして甲状腺機能異常が知られている. とくにDown症候群は, 自己免疫疾患に罹患しやすいために, 橋本病やBasedow病に罹患しやすいことが報告されている. また非自己免疫性の甲状腺疾患である先天性甲状腺機能低下症 (congenital hypothyroidism : CH) も臨床でしばしば経験する. 甲状腺機能異常は, 大きく甲状腺機能低下症と甲状腺機能亢進症 (多くはBasedow病) に大別され, 甲状腺機能低下症は障害部位により中枢性と原発性に分けられる. 原発性甲状腺機能低下症は, 生まれつき甲状腺機能低下を呈するCHと自己免疫機序などにより後天的に機能低下となる後天性甲状腺機能低下症 (多くは橋本病) に分けられ, さらに重症度の点から顕性甲状腺機能低下症と潜在性甲状腺機能低下症 (subclinical hypothyroidism : SH) などに分類される. |
