| Abstract | 要旨: 気腫性腎盂腎炎は, 予後不良な重症尿路感染症である. 重症度分類とともに, 腎実質の破壊の程度, 全身状態, 生命予後のリスク因子などを十分に検討したうえで, ドレナージや腎摘の必要性を吟味し実践することが患者の生命予後の改善につながる. 腎膿瘍は, 急性腎盂腎炎治療中の薬剤抵抗例や尿路結石治療後の遷延する発熱症例には, 本疾患を念頭におくべきであり, 抗菌薬抵抗例やサイズが大きい症例には穿刺術を考慮する必要がある. フルニエ壊疽は, 気腫性腎盂腎炎とともに致死率の最も高い泌尿器領域の重症感染症である. 死亡率を減らし生命予後を改善させるためには, 早期診断とともに迅速な外科的介入が必要不可欠である. |
