| Abstract | 「要旨」腹水は, 非代償性肝硬変の重要な合併症の一つである. 従来からナトリウム排泄型利尿薬を中心とした治療が行われてきたが, 2013年に作用機序の異なる水利尿薬トルバプタンが適応となり, その治療方針に大きな変化がもたらされた. 肝性腹水に対する治療において基本となるのは腎機能の保護を念頭に置いた利尿薬の選択である. 腎機能の悪化を防止する観点から, フロセミドの使用量を少量にとどめ, 腎機能の良好な状態で早期にトルバプタンを導入する. 利尿剤使用時にはunderfillingと腎障害に注意を払うことが必要である. なお, 現時点でトルバプタンを用いた腹水治療が可能であるのはわが国のみであり, 欧米とは現状が異なっている. |
