Abstract | 「要旨」胆石性胆管炎の治療に関して, 一部の軽症例を除き内視鏡的経乳頭的ドレナージが推奨されている. また, 胆石性膵炎のうち胆管炎合併もしくは胆汁うっ滞所見を認める場合, 早期のERCP with ESTが推奨されている. 後半に胆管ドレナージ難渋例を提示した. 乳頭同定不能かつ胆管非拡張によりERCP・PTBD困難な胆石性胆管炎に対して, 細径針を用いたEUS下ランデブー法が有用であった. また, 胆管内の感染胆汁充満による早期胆管ステント閉塞例に対して, 専用デバイスを用いた胆管内洗浄がステント開存に有用であった. |
