| Abstract | 「要約:」膵癌は発症時から疼痛, 体重減少, 食欲低下などの症状を認め, 生活の質が低下するなど, 患者の不安が強いがん腫である. 骨格筋量の持続的な減少と機能障害である「がん悪液質」はがん一般の大きな特徴の一つである. 膵癌は主要ながん腫の中で非常に体重減少率が高く, がん悪液質の合併が多い. 膵癌においては, がん悪液質が発症前から進んでいることが報告されている. がん悪液質の状態に陥ると全身状態の低下をきたし, 生命予後を悪化させることから, 早期にがん悪液質への対策が必要である. グレリンは食欲促進, 抗炎症作用, 筋蛋白質分解の抑制, 筋蛋白質合成の促進などの作用があり, がん悪液質の複数の病態を改善すると考えられる. グレリン様作用をもつ経口薬アナモレリンは, 非小細胞肺癌, および大腸癌, 胃癌, 膵癌の消化器癌患者を対象とした臨床試験が行われ, 体重増加および食欲不振の改善の臨床効果が認められている. 今後一般臨床への導入が期待されている. |
