| Abstract | 要約:超音波内視鏡下胆道ドレナージ術として, EUS-hepaticogastrostomy(EUS-HGS)ルートから一期的あるいは二期的に胆管狭窄部へステントを留置する方法である超音波内視鏡下順行性ステント留置術(EUS-AS)の有用性が報告されている. EUS-ASの適応は, EUS-HGSと同様であり, 経乳頭的胆管ドレナージ困難症例である. 基本的に悪性胆道狭窄へEUS-HGSを選択する場合, 一期的にEUS-ASを行い, 胆管が非常に細く手技に難渋した症例や, 遠位胆管狭窄部をガイドワイヤーが突破しづらい場合には. 胆汁漏の危険性からまずEUS-HGSを行い, 二期的にEUS-ASを行っている. 当科で行っているEUS-ASを実際に使用するデバイスと方法について概説する. |
