Abstract 「要約:」高齢化社会において, 偶発的に膵嚢胞性病変が診断される機会が多くなった. 膵嚢胞性病変を評価する際, 手術適応かどうかの判断が重要であるが, 組織診断は困難なことが多く, 画像診断によるところが大きい. 膵管内乳頭粘液性腫瘍(IPMN)は高齢者に多く, その治療方針は国際診療ガイドラインに沿って決定される. 通常, high-risk stigmataを有するIPMNは切除対象となるが, 膵切除術は高侵襲であり, 高齢者に対しては慎重な判断が求められる. 切除対象とならないIPMNの場合でも, IPMN自体の悪性化や併存膵癌発症リスクがあるため長期的な経過観察が必要だが, 至適観察期間は明確ではない. 高齢者のIPMNに対して治療方針を決定する際には, 益と害のバランスを考慮し, 十分なインフォームドコンセントの上でなされるべきである.
