| Abstract | 「はじめに」 肩腱板断裂とは肩甲上腕関節に付着する腱板筋の腱性部分が断裂し, 腱線維の連続性が断たれた状態と定義される. 特に腱板筋の退行変性を基盤とした中高齢者に好発することが知られ, 腱板筋収縮による応力集中, 肩峰との機械的な衝突および摩擦, 外傷等, さまざまな要因が重なって発症すると考えられており, スポーツ選手にも多く発症する. 肩腱板断裂は肩の疼痛や機能障害につながる肩関節の代表的疾患であり, 日常診療において遭遇する機会が非常に多い疾患である. また肩腱板断裂は断裂の程度に応じて, 完全断裂(全層性断裂)から不全断裂(部分断裂)に分類される. 不全断裂はさらに滑液包面断裂, 腱内断裂, 関節面断裂に分類される. また断裂の大きさにより, 小断裂, 中断裂, 大断裂, 広範囲断裂に分けられている. そのため, どのような分類に区別されているかを評価することは治療方法選択の一助となる. |
