Abstract | 「疾患の特徴」外傷性脊髄損傷は, 転倒や転落, 交通事故等によって脊髄が損傷されることにより, 四肢, 体幹または両下肢の運動麻痺, 感覚障害等を生じる. また自律神経障害により, 膀胱直腸, 循環器等, 運動器以外の種々の障害も生じる. 病理学的には, 局所の機械的損傷 (一次損傷) に続き, 虚血, 炎症, 浮腫等による二次損傷が拡がる. わが国における1990〜1992年の疫学調査では, 外傷性脊髄損傷の頻度は100万人あたり40.2人で主な受傷原因は交通事故 (43.7%) , 高所転落 (28.9%) , 平地転倒 (12.9%) で, Frankel Aの割合が25.8%と最多であった. 近年では高齢者の転倒による不全麻痺の割合が増加し, わが国で最も高齢化が進んでいる秋田県における2012〜2016年の調査では, 高齢化率は30.4〜34.6%で, 発生頻度は100万人あたり86人, 平均年齢65.9歳, 頸髄損傷89.8% (うち非骨傷性損傷65.5%) , Frankel D 53.5%で受傷原因の最多は平地転倒 (32.1%) であった. |
