| Abstract | 「概要」多くの原因不明の小児関節炎の総称である若年性特発性関節炎(JIA)のうち全身型JIAは1896年に英国小児科医のStillによって報告されたことに始まり, 当初はスチル病(Still's disease)と称された. 1971年にこれも英国のBywatersがスチル病類似の臨床像を持つ成人14例を報告したことが成人発症スチル病の始まりである. 現在では小児例も成人例も病態はほぼ同じと考えられ, 成人期発症のみならず小児期発症で成人期まで遷延したものも含め成人スチル病と定義される. 本症は発熱, 関節症状, 皮疹を3主徴とする原因不明の全身性炎症性疾患である. 発熱は典型例では弛張熱の熱型を示すが, 成人の不明熱をみたときに鑑別すべき疾患の1つである. また皮疹は典型的には有熱時に生じサーモンピンク疹と呼ばれる. 3主徴の他に肝障害, 脾腫, リンパ節腫脹なども生じ, 血清フェリチン値が著増することも知られる. |
