| Abstract | 「要旨」2010年に日本癌治療学会の制吐薬適正使用ガイドラインの初版が発刊され, 2015年10月に第2版の改訂版が発刊された. さらに2018年Web改訂版ver. 2.2が作成され, 今回その改訂のポイントを解説する. 2018年の改訂では2015〜2018年のエビデンスが追加されている. ポイントはMECにおいてカルボプラチンを含むレジメンと非カルボプラチンレジメンに分けて, カルボプラチンを含むレジメンをHECと同様にNK1受容体拮抗薬, 5-HT3受容体拮抗薬, ステロイドの3剤を推奨した. また, オランザピンが制吐薬としての公知申請により, 2017年6月から他の制吐薬との併用において成人では5mgを1日1回経口投与 (患者の状態により1日10mgまで増量可能), 最大6日間を目安として承認された. さらにMECに対してNK1受容体拮抗薬を使用しない場合, パロノセトロンを併用するという条件下でday 2〜3のステロイドの省略が可能であるとされた. |
