| Abstract | 「要旨」がんに伴う病態の一つにがん悪液質が知られている. がん種によってその頻度は異なるが, 消化器がんや肺がんでは比較的高い頻度で認められる. がん悪液質に特徴的な症状として体重減少の他, 食欲不振や倦怠感がある. がん悪液質の診断については2011年にEPCRCよりコンセンサスレポートが出版され, 体重減少の程度に基づいた診断基準が現在広く用いられている. がん悪液質の原因についてはがんに伴う炎症が重要な役割を果たしているが, 近年の研究ではがん細胞から直接産生される蛋白の分解を促進する因子などの関与が明らかになりつつある. 現時点では確立された薬物療法はないが, グレリン様の作用を示すアナモレリンに注目が集まっている. アナモレリンは食欲中枢の刺激による食欲亢進および筋蛋白の同化作用により, 臨床試験においてがん患者の除脂肪体重を有意に増加させることを示した. 今後はがん悪液質に対して薬物療法だけでなく, 栄養療法や運動療法を含めた包括的な治療戦略の確立が求められている. |
