| Abstract | 「要旨」 心筋炎は鑑別診断として念頭に置けるか, 状況に応じた循環動態を管理できるかの二点が重要である. 心電図と心筋トロポニンが診断の柱となる. 心電図でのST-T異常は心筋炎の多くで出現する. また, 心筋トロポニン陽性は心筋炎を疑い, 循環器医にコンサルトする. 心筋炎と診断された場合は, 積極的に心臓カテーテル室とCCUが常備された高度循環器施設へ搬送すべきである. irAE心筋炎では, ステロイドパルスをはじめとする免疫抑制療法を積極的に行う必要がある. |
