| Abstract | 「はじめに」メラノーマの治療を中心に飛躍的な進歩を遂げた2010年以降の薬物療法は, 今なお治療成績の向上をめざした開発競争のなかにある. 殺細胞性抗癌剤の時代から分子標的治療や免疫療法へと移行しており, 皮膚がんにおける分子標的治療としては, メラノーマのBRAF遺伝子変異例を対象としたBRAF阻害薬とMEK阻害薬の併用療法がよく知られている. 免疫療法においては, メラノーマやメルケル細胞癌, 有棘細胞癌をはじめとした上皮系皮膚悪性腫瘍におけるPD-1, PD-L1の他, CTLA-4やLAG-3を標的にした新規治療法が登場している. 疾患名に準拠した薬物療法とエビデンスという観点以外に, 個別化治療ともいわれることがあるがん遺伝子パネル検査結果に基づく薬物療法選択が同時にあり, 今後がん遺伝子パネル検査が早期に実施されるようになることで, 治療大系は大きく変化することになる. |
