| Abstract | 「はじめに」 近年, 免疫療法ががんの薬物療法を席巻しているが, Atezolizumab+Bevacizumab療法が保険承認となり, 肝細胞癌にも免疫療法の時代が到来した. SHARP試験で初めて肝細胞癌に対し延命効果が示されたSorafenibが2009年に登場して以来, 多くの薬剤が1st lineの第III相試験に挑んだが, 長らくSorafenibに対して優越性を示す薬剤は見いだされなかった. このような中, 2019年, Atezolizumab+BevacizumabはIMbrave150試験において, Sorafenibに対し全生存期間, 無増悪生存期間およびquality of life(QOL)の点で優越性を示し, 今後, 導入困難な状況でなければ, 多くの症例で1st lineとして導入されるものと予想される. 肝細胞癌に対する免疫療法として行われたNivolumabおよびPembrolizumab単独の第III相試験では, 臨床的には免疫チェックポイント阻害剤(ICI)の有効性が示唆されたものの, 統計学的には有効性を示すことはできなかった. |
