| Abstract | 「はじめに」本稿で扱うのは, 進行・再発胃癌に対する薬物療法である. 化学療法とも言い換えられ, 通常の殺細胞性薬剤だけでなく, 分子標的薬剤や免疫チェックポイント阻害剤も含まれる. 新薬開発およびそれらの臨床活用により, 日進月歩, 治療成績も向上しつつあるが, 現時点では完全治癒は困難であり, 癌に随伴する臨床症状の改善および生存期間の延長が治療目標となっている. 胃癌治療においては, 「胃癌治療ガイドライン」が日常臨床の指針となっている. 2021年7月に「胃癌治療ガイドライン」が改訂され第6版となった. その改正点も踏まえて殺細胞性薬剤および分子標的薬剤が中心の内容となる. 「(I) 薬物治療の適応と境界症例」胃癌の薬物療法の適応について, 「胃癌治療ガイドライン第6版」では, 「切除不能進行・再発症例, あるいは非治癒切除(R2)症例で, 全身状態が比較的良好, 主要臓器機能が保たれている場合は化学療法の適応となる. |
