| Abstract | 「Headline」・潰瘍性大腸炎の診断では, 下痢・血便といった症状が数週間〜数か月は続いていることを確認することが, 感染性腸炎との鑑別のために重要である. そのうえで, 下部消化管内視鏡検査で直腸からの連続性かつびまん性の炎症を確認する. ・Crohn病では, 縦走潰瘍, 敷石像といった内視鏡所見を正しく理解することが, 診断には重要である. 非乾酪性類上皮細胞肉芽腫はCrohn病に特異的な病理組織所見であるが, 検出率は40%程度であることに留意が必要である. ・炎症性腸疾患の治療選択肢は近年急増しているが, 基本に忠実に治療を進めることが大事である. |
