| Abstract | 「Headline」・COVID-19ワクチン接種後の肉眼的血尿はIgA腎症/IgA血管炎患者において女性優位に起こりやすく, mRNAワクチン接種後にのみ起こることが明らかになった. ・ワクチン接種後に肉眼的血尿を認めた症例では, 腎病理学的に慢性病変は少なく, 腎機能と尿所見の変化はあっても一過性であった. ・mRNAワクチンにより肉眼的血尿が生じた機序はいまだ不明であるが, 診断されたIgA腎症は新規発症というよりも, 潜在化していたIgA腎症がワクチン接種により顕在化した可能性が高い. IgA腎症は世界的に最も多い原発性慢性糸球体腎炎で, わが国でも発症頻度はきわめて高く, 大部分が血尿を初発症状とし, 病態の進行に伴って蛋白尿を呈しながら, 未治療の場合15〜20年の経過で約40%が末期腎不全に移行する予後不良の疾患である. IgA腎症患者のなかには扁桃炎や上気道感染により肉眼的血尿が出現する症例や尿所見異常が増悪する症例がみられ, また口蓋扁桃摘出術で腎症が改善する症例がみられることより, 粘膜免疫が病態に重要な役割を果たしていると考えられている. |
