| Abstract | 「はじめに」フレイルとは, 加齢に伴うさまざまな臓器機能低下によって外的なストレスに対する脆弱性が亢進した状態と定義される. フレイルには, 高齢期の筋力低下や低栄養などの身体的な問題だけでなく, 認知障害やうつなどの精神・心理的問題, 独居や閉じこもり, 経済的困窮などの社会的な問題が含まれ, 将来の転倒, 日常生活機能の障害, 入院, 生命予後などのさまざまな不良な転帰につながる病態である. さらに, 近年では, フレイルと認知障害や認知症との関連について検討したメタ解析において, フレイルは認知障害および認知症のリスクを高めることが明らかにされており, フレイルは要介護および認知症予防の観点からも重要である. 本稿では, フレイルと認知障害および認知症との関連について, フレイルの中核をなすサルコペニアや体重減少を含む栄養状態の観点から概説したい. 「1. フレイルサイクルとサルコペニア」フレイルは多面性をもつ概念であるが, そのなかでも身体的フレイルに関する研究が多く報告されている. |
