| Abstract | 炎症性腸疾患(IBD)は慢性腸炎であるが, その鑑別に同様に慢性炎症をきたす膠原病に伴う慢性腸炎がある. 腸炎として代表的なものに, 全身性エリテマトーデス(SLE)にみられる血管炎に起因したループス腸炎, 関節リウマチにみられるアミロイドーシス, 結節性多発動脈炎・リウマトイド血管炎による虚血性腸炎, IgA血管炎, 顕微鏡的多発血管炎等の血管炎に起因する腸炎, あるいは高安動脈炎に合併するIBD様腸炎が報告されている. その他ベーチェット病の腸管病変や膠原病治療中の薬剤に起因した腸炎, 免疫抑制状態における感染症による腸炎にも注意が必要である. これらの腸管病変は膠原病本体の診断, 治療においても重要となる. 慢性腸炎の鑑別において膠原病に伴うものを念頭に置いておくことが重要である. |
