Abstract | 小児期発症の炎症性腸疾患(IBD)患者数は世界中で増加の一途を辿っている. とくに近年は, 診断時年齢の低下があいついで報告されており, 今後6歳未満発症のIBD(VEO-IBD)もますます増加していくと予想される. 小児IBDの特徴は, (1)重症・難治例が多い, (2)成長障害をきたす, (3)非典型的が多い, そして(4)IBDのなかに単一遺伝子疾患に合併するIBD(monogenic IBD)が紛れていることである. monogenic IBDの新規原因遺伝子の報告があいつぎ, 現在ではその数は80を超える. monogenic IBDは典型的なIBDとは治療法が異なることがあり, 患者の病態に合わせたprecision medicineをおこなうためにmonogenic IBDを疑い遺伝子検査を経て診断することが重要である. |
