| Abstract | 「Summary」トルソー症候群とは, 悪性腫瘍による凝固能亢進状態を基盤とした静脈血栓塞栓症(VTE)および非細菌性血栓性心内膜炎(NBTE)などによる全身性動脈塞栓症を指し, 栄養障害, 脱水, 薬剤性血管炎などによる血栓症をも包含するがん関連血栓症(CAT)の一つと理解される. 脳梗塞合併例では, 頭部MRI画像よりNBTEなどからの心原性塞栓症が推定され, 悪性腫瘍として, 肺がん, 膵がん, 胃がん, 卵巣がんなどの腺がんが多いことから, CA19-9やCA125などのムチンが凝固能亢進に関与している可能性がある. 再発予防治療は未だ確立していないが, VTE合併例では, 低分子量ヘパリン皮下注(わが国では未承認)あるいはXa阻害薬が用いられる. |
