| Abstract | [Key Points] ●終末期心不全における呼吸苦に対してモルヒネの有効性安全性が報告されている. ●終末期心不全では腎機能低下症例が多く, モルヒネの有害事象に注意が必要である. ●重度の腎機能低下症例に対してはオキシコドンの投与を検討する. ●オピオイドの投与目的, 用量調節, 評価有害事象などについて関係する多職種が共通認識をもって対応することが望ましい. ●呼吸苦改善目的のオピオイドの投与は適応外使用であること, 一般的に認知されていないことから, 患者・家族へ丁寧な説明が必要である. |
