| Abstract | [Key Points] ●細菌性髄膜炎に対してデキサメタゾンを抗菌薬前に投与することで聴力と神経学的予後の改善が期待できる. ●成人は肺炎球菌性髄膜炎に対するデキサメタゾン投与によって生命予後改善が期待できるが, 小児はインフルエンザ菌b型による髄膜炎における聴力予後の改善にとどまる. ●デキサメタゾン投与終了後に再発熱を認めることが多い. ●培養が陰性の場合は鑑別となるウイルス性髄膜炎, がん, 膠原病に留意する必要がある. ●成人の細菌性髄膜炎疑い患者に対してはデキサメタゾン投与が推奨されているが, 小児は総合的判断に委ねられている. |
