| Abstract | [Key Points]●アンジオテンシン受容体ネプリライシン阻害薬(ARNI)は, 基本的にはACE阻害薬, ARBからの切り替えとして用いる. ●ARNIの主な対象は左室駆出率が低下した慢性心不全患者である. ●投与前後の心不全の経過フォローにはNT-proBNPが有用である. ●併存疾患や血圧を考慮して, ARNI使用のタイミング, 他の新規慢性心不全薬と使い分けを考える. ●導入後の腎機能障害, 高カリウム血症, さらに血圧低下には注意を要する. |
