| Abstract | 「はじめに」SGLT2阻害薬に関連した糖尿病性ケトアシドーシス(diabetic ketoacidosis: DKA)では, 血糖が正常もしくはわずかな増加にとどまることがある. 在宅での血糖値上昇が乏しいことで, そもそも救急外来への来院が遅れることも多いが, 患者の約50%が診断の遅れを経験しているとも報告されている. 「[I] 正常血糖ケトアシドーシスとは」正常血糖ケトアシドーシス(euglycemic DKA)とは, 1973年に血糖値 < 300mg/dLのDKAとして初めて報告され, その後, SGLT2阻害薬に関連しても発症することが報告された. 以下のような診断基準が用いられている. [●相対的に血糖が正常(< 250mg/dL) ●アシドーシス(pH < 7.30, HCO3- < 18mEq/L) ●ケトーシス(血清β-ヒドロキシ酪酸 > 3mmoL/Lが望ましい, 血清アセト酢酸または尿ケトンも使用可能)] |
