| Abstract | 「Point」●高齢者の4人に1人は圧迫骨折があり非常にコモン. ●診断はX線写真が基本. CTの読影レポートも見落さない. ●保存的加療にもさまざまな種類があり, 個別化が重要. ●骨粗鬆症として治療開始後, 5〜10年以上継続してフォローする. 「はじめに」胸腰椎圧迫骨折は, 腰痛のなかでも原因が特定しやすい疾患であり, 有病率も高いため, 読者の皆さんにもよく馴染みのある疾患ではないだろうか. 日本における圧迫骨折の有病率は, 60代で25%, 80代以上で41%という報告がある. 高齢化に伴い状況はさらに加速していると思われ, 高齢者の4人に1人以上は圧迫骨折があると考えられる. 腰痛を主訴に来る患者だけでなく, 画像で偶発的にみつかることも多く, 内科・外科問わず熟知しておくべき疾患の1つだろう. ここでは, 圧迫骨折の診断に必要な病歴や画像検査, 保存的治療および手術療法, さらに骨粗鬆症の治療も含めて, エビデンスやまだ不明確な点も踏まえながらまとめていく. |
