| Abstract | 「要旨」過去3年半に当科で経験した急性巣状細菌性腎炎(acute focal bacterial nephritis; 以下AFBN)16例について後方視的に検討した. 性別は男児6例, 女児10例, 年齢中央値は4.1歳, 診断日の中央値は入院3.0日目であった. 治療開始から解熱までの中央値は41.5時間と長く, また, 経過中のCRP最高値の中央値は7.3mg/dLと高値であった. 膿尿陰性が4例, 尿培養陰性が4例あり, 腹部超音波検査で異常を認めたのは2例のみであった. 膀胱尿管逆流症の合併は9例あり, 腎の瘢痕・萎縮も9例で確認し, 1例においては腎機能低下を認めていた. AFBNは診断自体に否定的な意見もある. しかしながら, AFBNを見逃さないことで, 十分な治療期間を設定でき, 治癒後の泌尿器科的精査ならびに長期的な腎機能のフォローへとつなげることができる可能性があるため, 急性期の診断は重要であると考える. |
