| Abstract | 「はじめに」近年の炎症性腸疾患の治療目標は粘膜治癒が推奨され, 早期に寛解導入し, 粘膜治癒が得られると予後が良い. また, 成人と異なる小児の治療目標として, 身長や体重などの身体発育と二次性徴を含めた成長をcatch-upさせ, 同時に, 長期的に消化管だけでなく, 管外症状も含めた合併症を予防することも治療目標で, やはり病初期の早期の寛解導入が合併症の予防にも有効である. 「I. 潰瘍性大腸炎」寛解導入と寛解維持療法に分けて, 重症度と罹患範囲に応じて治療を選択する. また, ステロイド抵抗例や依存例などの難治例の治療は専門医と相談することが望ましい. 「1. 寛解導入療法」「1) 直腸炎型」直腸炎型は軽症例が多い. 原則として寛解導入・維持療法のいずれにも5-アミノサリチル酸(5-ASA)製剤の局所療法(坐剤または注腸薬)が勧められる. この治療で奏効が得られない場合にはステロイドの坐薬や注腸療法が選択される. |
