| Abstract | 「緒言」日本小児リウマチ学会は, 若年性特発性関節炎(juvenile idiopathic arthritis; 以下, JIA)の適切な診断と標準的な治療について, 本邦の一般小児診療に携わる医師のために, 「初期診療の手引き」を作成し, 2015年には改訂版を刊行した. これにより, 成人の関節リウマチの類縁疾患と考えられるJIAに罹患した患児の診断・治療に際して, 早期診断法および早期治療法を一般に供することにより関節炎の早期消褪を目指し, 最終的には炎症関節の予後の改善を図ることが可能となった. 同時にこの「初期診療の手引き」は, ここに記された治療法によっても関節炎症の抑制に至らない病児を, 次の段階の治療法へとトリアージする役割ももつ. 近年, 生物学的製剤(トシリズマブ, エタネルセプト, アダリムマブ, アバタセプト, カナキヌマブ)が小児においてもわが国で適応拡大が認められたため, 一般小児診療に携わる医師がJIA患児の治療を行う際には, 小児リウマチ専門医との連携を密に行い, 従来の治療では無効であることが判明した場合には時期を違えずに生物学的製剤による治療へ移行することが要求されている. |
