| Abstract | 「要旨」過去5年間に小児亜急性壊死性リンパ節炎(subacute necrotizing lymphadenitis;SNL)と診断, 治療を行った8症例, 計9エピソードの臨床像について検討した. 症例は年齢が7〜15歳, 男児5例, 女児3例で, 1例に4年後の再発を認めた. リンパ節生検の実施は1例のみで, そのほかは生検を実施せずSNL疑い例(臨床診断例)として治療, 経過観察されていた. 9エピソードの特徴は, (1)持続する発熱と片側有痛性リンパ節腫脹,(2)病変リンパ節は最大3cm未満と小さいこと, (3)白血球減少, 軽度の血小板数低下, (4)血清LDH, フェリチン高値, (5)骨髄での血球貪食像, であった. 治療は全例にステロイドが著効した. SNLは菊池, 藤本により特異的な病理組織像をとるリンパ節炎として報告され, 診断に生検が必須である. しかし, 今回の検討では侵襲性の問題点から, 生検なしでの診断を求められた症例が多かった. このため今後も小児SNLの鑑別診断に役立つ臨床所見, 検査所見, 画像所見の検討が必要と思われた. |
