| Abstract | 「はじめに」急性散在性脳脊髄炎(acute disseminated encephalomyelitis: ADEM)は, 自己免疫学的機序による中枢神経の炎症性脱髄疾患であり, 意識障害をはじめとする脳症様症状を示す疾患である. 小児に発症しやすく, 一般に単相性で機能的予後の良い疾患群である. ADEMの診断の際には, 多発性硬化症(multiple sclerosis: MS)や抗ミエリンオリゴデンドロサイト糖タンパク抗体関連疾患(myelin oligodendrocyte glycoprotein antibody-associated disease: MOGAD), 視神経脊髄炎スペクトラム障害(neuromyelitis optica spectrum disorder: NMO-SD)など, 再発性の炎症性脱髄疾患の初発の可能性があり, 治療, 予後に関わるため鑑別を行うことが重要である. 「1. 病因」発症メカニズムや遺伝的要因の詳細は解明されていないが, 外来抗原による免疫応答により生じるとされる. |
