| Abstract | 「はじめに」骨粗鬆症治療の目的は骨折の予防である. その目的を達成するためには, 治療対象となる患者を正しく特定し, 骨折抑制効果の確かな薬剤を適切に用いることが重要である. 骨折予防が必要とされるのは50歳以上の男女, とりわけ閉経後の高齢女性で骨折リスクの高い患者である. 窒素含有ビスホスホネート製剤の代表であるアレンドロネート登場以降に承認された骨粗鬆症治療薬は, 全て骨折抑制効果が実証されたものである. それらの薬剤では, プラセボもしくは実薬を対照とした2〜3年間の二重盲検無作為割り付け試験の結果で骨折抑制効果が確認されている. しかしながら, 作用機序や薬剤特性はそれぞれ異なっており, 個々の患者に適した薬剤を選択することが大切である. 骨折抑制効果は短期間の治療で得られるものではなく, 少なくとも1年以上の確実な治療継続が必要とされる. とりわけ大腿骨近位部骨折の予防には長期にわたる治療継続が求められる. |
