| Abstract | 「はじめに」 超高齢化社会の到来に伴い, わが国における高血圧の罹患患者数は4,300万人(成人の2.5人に1人)といわれている. 高血圧は糖尿病や脂質異常症, 慢性腎臓病などといったさまざまな生活習慣病と密接に関わり, レニン-アンジオテンシン系や交感神経系, 炎症・酸化ストレスなどを介し, その病態が進行していく. 高血圧は心筋梗塞や心不全, 脳卒中などの脳心血管病の危険因子であるため, その適切な治療は脳血管病の予防につながる. ネプリライシン阻害薬はNa利尿ペプチドの血中濃度を上昇させることによるさまざまな薬理作用のある新規降圧薬として注目されてきた. ネプリライシン阻害薬単独では有意な降圧効果が証明されなかったものの, その後, アンジオテンシン変換酵素(ACE)阻害薬やアンジオテンシン受容体拮抗薬(ARB)との配合薬の開発が進められた. 特にアンジオテンシン受容体ネプリライシン阻害薬(angiotensin receptor neprilysin inhibitor: ARNI)であるサクビトリル/バルサルタン(LCZ696)による大規模臨床試験では, その有用性が示されてきている. |
