| Abstract | 「はじめに」肩峰下インピンジメント症候群は1972年にNeerが初めて提唱した概念である. 肩を挙上することにより, 大結節, 棘上筋腱, 上腕二頭筋長頭腱が肩峰の前下縁と衝突し, 疼痛を引き起こす状態とされている. その背景にはさまざまな要因があり, 石灰沈着性腱板炎や腱板断裂などに続発する場合も少なくない. Mastenらはインピンジメントを引き起こす因子として構造的因子と機能的因子に分類している. 両因子ともに保存療法で一定の解決をみる可能性があり, 基本的には保存療法が選択される. 1987年にEllmanが関節鏡を用いて行うarthroscopic subacrominal decompression (ASD) を報告して以降, 保存療法に抵抗する症例に対してASDが単独もしくは関節鏡視下腱板修復術の際に併せて行われている. 手術により構造的因子が改善しているにもかかわらず, 術後にインピンジメント症状を訴える症例を経験する. |
