| Abstract | 「●はじめに」加齢とともに2型糖尿病が増える. 2019年の厚生労働省の国民栄養調査では, 70歳以上の男性の26.4%, 女性の19.6%は糖尿病が強く疑われる人である. 加齢による糖尿病患者の増加の原因はインスリンの追加分泌の低下とインスリン抵抗性の増大である. インスリン分泌低下は加齢に伴うβ細胞のテロメア長の短縮, 酸化ストレス亢進などが関与し, インスリン抵抗性は内臓脂肪増加, 骨格筋量低下, 身体活動量低下などが関係している. 認知機能障害, フレイル, 心不全, 低血糖などは75歳以上または80歳以上の高齢者で増加する. 65歳以上の糖尿病を高齢者糖尿病としているが, こうした老年症候群・併存疾患の合併が多くなる後期高齢者の糖尿病と老年症候群を合併した前期高齢者の糖尿病が「治療上注意すべき高齢者糖尿病」である. 「●高齢者糖尿病の特徴」高齢者糖尿病では口渇, 多飲, 多尿などの高血糖症状が出現しにくい. |
