Abstract | 「はじめに」現在行われている緑内障手術の術式は, 大きく分けて濾過手術, 房水流出路再建術, 瞳孔ブロックを解消する手術, 毛様体破壊術の4つに分類される. なかでも, 房水流出路再建術については線維柱帯切開術(トラベクロトミー)が代表的であったが, 近年は低侵襲化と時間短縮を図る手術として低侵襲緑内障手術(minimally invasive glaucoma surgery:MIGS)が主流となっている. MIGSは眼内から線維柱帯にアプローチすることで結膜への侵襲を与えず, 追加の緑内障手術の妨げにならない. また, 手術による合併症が少なく安全性が高い. このような理由から, わが国でも初回手術としてMIGSが適応される症例が劇的に増加し, 現在の緑内障手術の潮流となっている. 本稿では, 各種MIGSについて特徴を紹介する. 表2には, 各種MIGSについての特徴を簡単にまとめた. |
