| Abstract | 夜尿症 (NE) は5歳以上の小児の間欠的な夜間就寝中の尿失禁であり, 重症例では自然消失率が低く, 自尊心の低下などの心理的影響もある. 2020年の国際小児禁制学会の診療指針と2021年の診療ガイドラインの改訂により, 単一症候性NEだけではなく非単一症候性NEに対しても診療アルゴリズムが作成された. NEの原因については明らかになっていないが, 夜間多尿, 膀胱容量低下, 睡眠からの覚醒障害が関与する. 治療はNEの分類に基づき, 第一選択としてデスモプレシン製剤やアラーム療法があるが, 選択的β3受容体作動薬の有効性を示す報告が近年増えている. また, 標準的治療では難治な症例においては, 生活リズムや睡眠の乱れ, 神経発達症の合併などの要因も考慮する必要がある. |
