| Abstract | 「薬の使い方のポイント・注意点」・うつ病治療では, 薬物療法を開始する前に全例において支持的精神療法と心理教育を中心とした基礎的介入を行うことが重要である ・中等症以上のうつ病では薬物療法が必要であり, 新規抗うつ薬を鎮静系・非鎮静系に分類して使用することが臨床上有用である ・抗うつ薬は十分量・十分期間の投与が必要で, 初期用量 (少量) から効果と副作用に応じて漸増していくのが基本であるが, 服薬アドヒアランスを保つ意味でも患者に対して投与前からの説明が重要である ・悪心・嘔吐などの一般的な新規抗うつ薬の副作用だけではなく, 上部消化管出血やアクチベーション症候群にも注意する ・寛解達成後も初発例では6カ月以上, 再発例では2年以上の抗うつ薬の継続が必要であり, 中止する際は中断症候群に注意し慎重な漸減が望ましい |
