| Abstract | ◆薬の使い方のポイント・注意点◆・第一選択薬としては長時間作用型カルシウム拮抗薬, アンジオテンシン変換酵素阻害薬, アンジオテンシンII受容体阻害薬, チアジド系利尿薬のいずれかを用いる ・用量の増量よりも, 異なる作用機序の薬剤を少量ずつ併用するほうが降圧作用, 副作用の両面でメリットが大きい ・夜間や早朝の血圧が高い場合は夕食後や眠前の内服も検討する「1. はじめに」高血圧治療は, 薬物療法の完成度がきわめて高く, 現在使用可能な降圧薬を適切に組み合わせれば, ほとんどの症例で良好な血圧コントロールを得ることができる. その一方で, 治療抵抗性の代表的な原因の1つとして, 不適切な降圧薬の選択(組み合わせを含む)が処方医側の問題点として存在している. 「高血圧治療ガイドライン2019」(JSH2019)が2019年4月に発行された. 分類が刷新されて, 高血圧の範囲が広がったことに加えて, 家庭血圧や24時間自由行動下血圧に基づく診断基準が示された. |
