| Abstract | 「薬の使い方のポイント・注意点」・抗甲状腺薬の第一選択薬は, 小児期や成人男性, 非妊娠期の成人女性についてMMI(methylmercaptoimidazole)で, 開始後8週間は2週ごとに副作用のチェックをする ・妊娠初期(器官形成期)は, 無機ヨウ素とPTU(propyl-thiouracil)を選択する ・甲状腺機能低下症の場合は, T4製剤を開始し, 4週ごとに検査結果をみながらTSH基準値を目標に投与量を増減する 「1. 病態, 薬の作用機序」「1)甲状腺ホルモン製剤」甲状腺ホルモン製剤は2種類ある(T4製剤, T3製剤). レボチロキシンナトリウム(チラーヂン(R)S:T4)はリオチロニンナトリウム(チロナミン(R):T3)プロホルモンであるが, T4は体内で脱ヨウ素化され活性型のT3となる. 半減期はチラーヂン(R)Sが1週間, チロナミン(R)が1日, 最高血中濃度到達時間(Tmax)はチラーヂン(R)Sが6〜7時間, チロナミン(R)が1.5〜2.5時間である. 「2)抗甲状腺薬」抗甲状腺薬は, PTUとMMIの2種類がある. |
