| Abstract | (1)水利尿薬であるトルバプタンは, 既存のナトリウム利尿薬に抵抗性の体液過剰に使用する (2)水分制限, 塩分負荷のみでは管理できない低ナトリウム血症にトルバプタンを使用する (3)トルバプタン使用時は急激な血清ナトリウム濃度の上昇, 肝機能障害に注意する 「はじめに」ループ利尿薬やサイアザイド系利尿薬などのナトリウム利尿薬と比べると, バソプレシンV2受容体拮抗薬であるトルバプタンは初期研修医や後期研修医にとって使いづらいという印象をもたれやすい水利尿薬です. トルバプタンの適応には心不全・肝硬変による体液過剰, 常染色体優性多発性のう胞腎の進行抑制があり, 2020年からは抗利尿ホルモン不適切分泌症候群(syndrome of inappropriate antidiuretic hormone secretion:SIADH)における低ナトリウム血症が追加されました. |
