| Abstract | 「はじめに」近年, 日本では初産年齢の高齢化に伴い, 慢性疾患をもつ女性の妊娠が増加している. 関節リウマチ(RA)の診療においても, 周産期の管理は重要性を増してきている. 挙児可能年齢の患者の診療に際しては, 早い時期から患者と家族へ, RAと妊娠に関する情報を提供し, 将来の家族計画を共有することが重要である. そして妊娠前から妊娠中, 産後にかけて適切に薬剤を使用することが求められる. 2020年2月に発表された米国リウマチ学会によるリウマチ性疾患におけるリプロダクティブヘルスに関するガイドラインでも, 妊娠前からの十分なカウンセリングが推奨されている. RAと妊孕性については, 妊娠前, 妊娠初期のRA女性を対象としたオランダの大規模前向きコホート研究で, 年齢のほかに疾患活動性が高いこと, プレドニゾロン使用, 非ステロイド性抗炎症薬(NSAIDs)使用が妊孕性低下に関連すると報告されている. |
