| Abstract | 「はじめに」 心不全に対する薬物治療として, 収縮力の低下した心不全(heart failure with redued ejection fraction: HFrEF)においては, アンジオテンシン変換酵素阻害薬(ACEI), あるいは忍容性がなければアンジオテンシンII受容体type I阻害薬(ARB), β遮断薬, ミネラルコルチコイド受容体拮抗薬(MRA)の3クラスの薬剤が予後改善効果を有することから, 日本を含む各国のガイドラインにおいてその使用が推奨されてきた. 一方で, これら薬剤によりHFrEF患者の予後は改善してきたものの, その予後はいまだ不良であるといわざるを得ず, 新たな心不全治療薬の開発が強く望まれてきた. 近年新規作用機序を有する心不全治療薬として, hyperpolarization activated- and cyclic nucleotide-gated channel (HCNチャネル)阻害薬, アンジオテンシン受容体ネプリライシン阻害薬(angiotensin receptor neprilysin inhibitor: ARNI)が開発され, 慢性心不全患者に対する予後改善効果が示され, 各国のガイドラインでもその使用が推奨されるようになってきた. |
