| Abstract | 「はじめに」 近年, 心不全, 特に左室駆出率(LVEF)が低下した心不全(HFrEF)に対する薬物療法の新たなエビデンスが相次いで報告されている. Ifチャネル阻害薬であるイバブラジン, アンジオテンシン受容体ネプリライシン阻害薬(ARNI)であるサクビトリルバルサルタン, およびナトリウム・グルコース共輸送体2(SGLT2)阻害薬であるダパグリフロジンやエンパグリフロジンがHFrEF患者に対して有効であることが示され, HFrEF患者への治療方針が変わろうとしている. 本稿では, 新たな薬剤の位置づけを最新のガイドラインに基づいて概説する. 「HFrEF患者に対する従来の基本治療」 HFrEFの病態形成・進展に慢性的な神経体液性因子の活性化が重要な役割を果たしていることが知られている. 神経体液性因子の活性化は心筋リモデリングを引き起こし, さらなる心筋障害および心ポンプ機能不全をもたらす. |
